Apply for an Animal Health Certificate

Use this form to apply for an EU Animal Health Certificate (AHC) for your dog, cat or ferret. Fill it in a couple of weeks before you travel if you can. We will check the details against our records and ring you to book the appointment, which must fall within the 10 days before you enter the EU.

Vaccination record

We need to see three things from the vaccination card or pet passport. They are often on different pages, so take a separate photo of each:

The page with the pet's details: name, breed, sex, date of birth.
The microchip number, usually a sticker or a printed page.
Every rabies vaccination entry, with the vaccine sticker and the vet's stamp.

Please confirm The journey is not for sale or transfer of ownership of the pet (non-commercial movement). * The owner (or the authorised person) will attend the appointment with the pet. * I will bring the vaccination record card / pet passport and any previous certificates to the appointment. * I understand the certificate must be issued within 10 days before entry to the EU, and that rabies vaccination, microchip and (for some countries) tapeworm treatment rules apply. *

We use these details only to prepare your pet's certificate and to contact you about the appointment. They are kept with your pet's clinical record.
